Common questions

Is at-home ketamine therapy safe?

At-home sublingual ketamine is lower-dose than clinic IV or IM ketamine and has a strong safety record when you follow the program rules: a sober sitter, no mixing with other CNS depressants, a stable and private home environment, and honest medical history disclosure during intake. The risk profile is not zero. Do not start an at-home program if you have untreated high blood pressure, recent cardiac events, active substance use disorder, or untreated psychosis in your personal or family history.

Does insurance cover at-home ketamine?

Almost never. At-home sublingual ketamine is off-label, so commercial insurance does not cover it. HSA and FSA funds usually work because the program is run by a licensed clinician. If insurance coverage is your priority, Spravato (esketamine) at a REMS-certified clinic is the covered path, not at-home ketamine.

How is at-home ketamine different from in-clinic IV ketamine?

In-clinic IV ketamine is a higher dose, delivered intravenously, in a monitored setting over 60 to 90 minutes. Effect onset and offset are tightly controlled. At-home sublingual ketamine is a lower dose, slower onset and offset, and relies on you providing the safe setting. Clinic IV has the strongest off-label evidence base. At-home has scale, convenience, and lower per-session cost. Different tools for different people.

Can I combine at-home ketamine with antidepressants?

Some combinations are routine (most SSRIs are fine with adjusted expectations), some are problematic (MAOIs, lithium at certain doses, benzodiazepines taken close to a session). This is what the telehealth intake interview is for. Never start, stop, or adjust a psychiatric medication without the supervising clinician. If your current prescriber is separate from the ketamine program, get them talking.
