Is IV ketamine covered by insurance?
Rarely at the drug level, sometimes at the administration level. Anthem BCBS (policy BEH.00014), Empire BCBS, some Highmark plans, and select Tricare policies cover IV ketamine for TRD. Most other plans do not. HSA/FSA are eligible for out-of-pocket payments.
How many sessions do I need?
A standard induction series is 6 sessions over 2-3 weeks. Response rates in TRD average 50-70% after induction. Maintenance frequency is individualized, typically one session every 2-6 weeks.
Is ketamine safe?
In a monitored clinical setting with appropriate screening, serious adverse events are rare. Risks include transient blood pressure increase, dissociation, nausea, and rarely, bladder irritation at high chronic doses.
