Cardiac clearance is non-negotiable

Ibogaine prolongs the QT interval. Every credible program below requires a 12-lead ECG, basic echocardiogram, liver panel, and medication reconciliation before admission. If a center offers to skip these or lets you book without them, that is the signal to walk away. Clinical deaths in this category have almost always involved skipped cardiac screening, co-use with opioids or stimulants, or undisclosed cardiac history.

Three established nonprofits run annual scholarships that cover 50 to 100 percent of retreat cost for vetted applicants. They typically work with Ambio, Clear Sky, and a handful of other partner clinics. Apply 4 to 6 months before your target retreat date.

Common questions

Is ibogaine legal for US veterans?

Ibogaine is a Schedule I substance in the US - you cannot take it on US soil legally. Every program listed here is based outside the US (Mexico or Costa Rica) and operates under that country's medical framework. A licensed US physician cannot prescribe it, but they can write the cardiac clearance letter you need before a retreat admits you.

Why do all these programs require a cardiac workup?

Ibogaine prolongs the QT interval on an ECG. In patients with pre-existing cardiac conditions, that prolongation can trigger a fatal arrhythmia. Every credible program requires a 12-lead ECG, basic echocardiogram, liver panel, and a medication reconciliation before they will admit you. If a center does not require this, do not go.

Will the VA pay for ibogaine treatment?

No. The VA cannot cover a Schedule I substance or off-shore medical care. However, the Heroic Hearts Project, Veterans of War, and the Helmet Group each run scholarships that can cover a significant portion of costs at partner retreats. Ambio and Clear Sky in particular have partnered with these programs. The 2024 Stanford ibogaine outcomes paper on Special Operations veterans may accelerate VA or DoD-funded clinical trial access, but retail coverage is not on the near horizon.

Ibogaine vs psilocybin for TBI - which is better?

Different targets. Psilocybin is a classical serotonergic psychedelic with strong acute antidepressant data. Ibogaine is a non-classical atypical psychedelic that appears to have a dual effect - acute trauma reprocessing plus a post-treatment neurotrophic window that may be uniquely relevant to TBI recovery (see the Stanford 2024 paper on combat veterans). For a veteran with confirmed TBI plus PTSD plus opioid exposure, ibogaine is often prioritized first. For depression or anxiety alone with no TBI or substance profile, psilocybin at a licensed Oregon or Colorado program is easier, safer, and cheaper.

How long does the full process take, including travel?

Plan for 7 to 14 days total. Most programs are 5 to 7 days on-site. Add 2 to 3 days post-retreat where you should not drive, fly long-haul, or return to work. Cardiac workup and paperwork happens 3 to 6 weeks before travel. Total calendar time from decision to back-to-work is usually 8 to 12 weeks.

What happens if something goes wrong medically?

The top-tier centers (Ambio, Beond) have on-site MDs, crash carts, continuous cardiac telemetry during the dose, and documented evacuation protocols to tertiary hospitals in Tijuana, Mexico City, or Cancun. Smaller operators may rely on local ambulance and hospital transfer. Ask for the emergency protocol in writing before you pay a deposit.
